A £100.00 one-off application fee plus pro rata of the yearly subscription fee of £363.00 is payable.

Requirements
Doctors (option 1)
- Must have current full GMC / IMC registration (unrestricted i.e. no conditions, warnings or APS) with a licence to practice and/or be registered with the Irish Medical Council (minimum 12 months)
- Evidence of completion of three years General Professional Medical Training after satisfactory completion of FY1 and FY2
- Be able to show at least 12 months experience working within an aesthetic environment
- Provide two professional references both from individual UK registered healthcare professionals who have known you in a professional capacity for at least two years or more (reference form).
- Full and up-to-date CV detailing all aesthetic work
- Proof of valid medical insurance/indemnity, a copy of your insurance certificate showing a minimum cover of £5million for non-invasive cosmetic procedures and £10million for invasive surgical procedures must be provided. The documentation must include your name on the policy.
- Proof of all aesthetic qualifications & training (including certificates).
- Enhanced DBS Certificate (no more than six months old on application) – please provide us access to your account on the DBS website to view your latest certificate
- A good quality, certified colour copy of the photo page of your passport. Making sure all four corners of the passport book are shown. For information on certification visit https://www.gov.uk/certifying-a-document
- A copy of a utility bill showing your home address (no older than three months)
- Evidence of previous Medical Appraisal (if applicable)

Doctors (option 2)
- Must have current full GMC / IMC registration (unrestricted i.e. no conditions, warnings or APS) with a licence to practice and/or be registered with the Irish Medical Council (minimum 12 months)
- Evidence of continuous practice in aesthetic medicine for a period of no less than three years and can demonstrate their general professional competence by submission of a portfolio of evidence in relation to general medical training, education and experience
- Provide two professional references both from individual UK registered healthcare professionals who have known you in a professional capacity for at least two years or more
- Full and up-to-date CV detailing all aesthetic work
- Proof of valid medical insurance/indemnity, a copy of your Insurance Certificate showing a minimum cover of £5million for non-invasive cosmetic procedures and £10million for invasive surgical procedures must be provided
- Proof of all aesthetic qualifications and training (including certificates)
