Dentists

  • All evidence MUST be sent to tracy.hilton@bcam.ac.uk via email or post in order to complete your application
  • Must have at least 4 years continuous GDC registration (unrestricted i.e. no conditions or warnings).
  • Evidence of substantial participation in aesthetic medical practise for a minimum of 12 months (this shall mean a minimum of 10 consultations per month over the past 12 months).
    You must provide a logbook containing details of your aesthetic consultations and treatments during this period. Your logbook must be signed by another healthcare professional who has knowledge of your practise.
  • Provide 2 professional references both from individual UK registered healthcare professionals who have known you in a professional capacity for at least two years or more.
  • Full and up to date CV detailing all Aesthetic work.
  • Proof of valid medical insurance/indemnity, a copy of your Insurance Certificate showing a minimum cover of £5million for non-invasive cosmetic procedures and £10million for invasive surgical procedures must be provided.
  • Proof of all aesthetic qualifications and training (including certificates).
  • Your Enhanced DBS Certificate (ORIGINAL copy of your DBS must be sent via post to Shorne Village Surgery, Crown Lane, Shorne, Kent DA12 3DY. This will be sent back to you once photocopied) must be no more than 6 months old on application. If you do not have an Enhanced DBS Certificate, you can obtain one by contacting CRB Disclosures on 01782 620000 who will make an application for you.
  • A good quality colour copy of the photo page of your passport/driving licence.
  • A copy of a utility bill showing your home address (no older than 3 months).
  • Evidence of most recent Annual Review of competence progression (if applicable).
  • You may also be asked to produce one of the following documents:-
    • If you qualified outside of the European Union then a P.L.A.B Certificate is required (unless exempted by the GMC)
    • If qualified within the European Economic Association (other than UK) then an I.E.L.T.S Certificate (with an overall score of 7.5 or over) may be required. Please see our published guidance on English Language requirements.
  • I wish to apply for Associate Membership of the British College of Aesthetic Medicine. I give permission for the College to seek information from any third party to verify my eligibility, standing or qualification for membership.

    I declare there are no past or pending cases against me by the GMC or any regulator in the UK or abroad.

    If I am accepted as an Associate Member, I agree:

    • To be bound by the Articles of Association of the Company and all Regulations adopted by the College.
    • To receive company notices, votes, polls and general correspondence in electronic form.
    • That the College may send or supply documents or information by making them available through its website.

    I understand that acceptance is at the absolute discretion of the Board.

    My application must be received in accordance with the Administrative & Member Conduct Regulations, Clause 4 which states “The Board shall only be required to consider applications for membership which are submitted not less than 60 days before the date of a Board meeting. The Board shall only be required to consider applications of any sort where the prescribed application form has been completed correctly in full, all required documents and evidence has been provided to the satisfaction of the Board and any specified application fee has been paid in full”.

  • Initial membership fee of £370.00 is NON-REFUNDABLE, therefore all applicants must take responsibility to provide all of the required evidence/documentation.
